Несмотря на то, что фармакологическое действие лекарственных средств хорошо известно и изучено, к сожалению, предсказать развитие лекарственной аллергии (ЛА) невозможно, так как в её развитии большое значение имеют индивидуальные особенности реактивности организма.</p><p>Цель — анализ клинической ситуации развития лекарственной аллергии для иллюстрации важности роли биотрансформации и химического строения лекарственных средств в её диагностике.</p></sec><sec><title>Методы</title><p>Методы. Исследование было проведено на пациенте после подписания информированного согласия и включало в себя сбора анамнеза, физикальный осмотр, скарификационные кожные пробы с ингаляционными аллергенами, установки периферического венозного катетера, провокационное тестирование.</p></sec><sec><title>Результаты</title><p>Результаты. Прик-тесты с: 0,9 % раствором NaCl (отрицательный контроль), раствором гистамина (10 мг/мл) (положительный контроль), амоксициллином + клавулановой кислотой (100 мг + 20 мг), ампициллином (100 мг/мл), цефалексином (50 мг/мл) — результаты положительные, цефтазидимом (20 мг/мл), кларитромицином (20 мг/мл) — результаты отрицательные; в/к тесты с: 0,9 % раствором NaCl (отрицательный контроль), цефтазидимом (2 мг/мл), кларитромицином (0,01 мг/мл) — результаты отрицательные; пероральный провокационный дозируемый тест (ПДТ) с цефуроксимом (кумулятивная доза – 250 мг), джозамицином (кумулятивная доза — 1000 мг) — результаты отрицательные. В результате проведённого кожного тестирования подтверждена ЛА на аминогруппу бета-лактамов; данных, касающихся ЛА, бета-лактамного кольца, клавулановой кислоты, макролидов не получено.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Relevance</title><p>Relevance. Despite the fact that the pharmacological effect of drugs is well known and studied, unfortunately, it is impossible to predict the development of drug allergies, since the individual characteristics of the body's reactivity are of great importance in its development. The goal is to analyze the clinical situation of the development of drug allergies to illustrate the importance of the role of biotransformation and the chemical structure of drugs in its diagnosis.</p></sec><sec><title>Methods</title><p>Methods. The study was carried out on the patient after signing informed consent and included anamnesis, physical examination, skin prick tests with inhalant allergens, installation of a peripheral venous catheter, and provocative testing.</p></sec><sec><title>Results</title><p>Results. prick tests with: 0.9 % NaCl solution (negative control), histamine solution (10 mg/ml) (positive control), amoxicillin + clavulanic acid (100 mg + 20 mg), ampicillin (100 mg/ml), cephalexin (50 mg/ml) — positive results, ceftazidime (20 mg/ml), clarithromycin (20 mg/ml) — negative results; intravenous tests with: 0.9 % NaCl solution (negative control), ceftazidime (2 mg/ml), clarithromycin (0.01 mg/ml) — negative results; oral PDT with cefuroxime (cumulative dose — 250 mg), josamycin (cumulative dose — 1000 mg) — negative results.
