Исследование чувствительности различных грибов к противогрибковым средствам in vitro по таким параметрам, как минимальные ингибирующие концентрации (МИК) и минимальные фунгицидные концентрации (МФК), характеризуются значительным разбросом межлабораторных результатов. Спектры активности пероральных противогрибковых средств существенно отличаются как in vitro, так и в клинических ситуациях. Это относится не только к различным классам грибов, таким как дерматофиты и дрожжевые грибы, но и к отдельным видам грибов и даже к отдельным штаммам одного и того же вида. Более того, фунгицидный эффект in vitro далеко не всегда коррелирует с фунгицидным эффектом in vivo. К сожалению, пока еще не существуют надежные и простые методы определения чувствительности in vitro, которые позволяли бы уверенно прогнозировать эффективность того или иного противогрибкового средства in vivo. Учитывая явно увеличивающееся разнообразие видов грибов, вызывающих поверхностные микозы, наиболее рациональным подходом представляется применение противогрибкового средства широкого спектра действия до того момента, пока не будет точно установлена этиология грибковой инфекции у конкретного больного. Помимо спектра действия, большую роль в эффективности противогрибкового средства in vivo играет его фармакокинетика. Для успешного лечения кожных грибковых инфекций способность проникновения противогрибкового средства в роговой слой кожи и/или в ногтевую пластинку является ключевым фактором, создающим необходимые условия воздействия на возбудителя – длительность воздействия и концентрацию, необходимые для подавления возбудителя.
