Этот протокол представляет собой получение пациентом тестовой дозы бусульфана, прогноз концентраций препарата в крови пациента в течение суток, индивидуализация режима дозирования на основе байесовского подхода. 29 детям была проведена операция по пересадки костного мозга после получения бусульфана. Индивидуальные значения фармакокинетических (ФК) параметров были оценены после получения пациентом тестовой дозы препарата (0,5 мг/кг) и использовались для индивидуализации режима дозирования в течение последующего 47дневного курса терапии. Дозы препарата рассчитывались с тем, чтобы достигалась желаемая терапевтическая цель, установленная как среднее значение площади под ФК кривой (AUC) за 6 часов в пределах 476 мг⋅ч/мл. Измерения концентрации бусульфана в крови проводились хроматографически. Для анализа ФК данных применялся пакет прикладных программ USC*PACK. Была оценена точность прогноза значений AUC на основе тестовой дозы. Проявления токсичности, рецидивы, выживаемость, частота и сроки возникновения веноооклюзивной болезни (ВОБ) оценивались и сравнивались с контрольной группой: пациенты изучаемой группы (группа А) сопоставлялись по определенным критериям с пациентами, получавшими стандартные дозы бусульфана (группа Б). Дозы бусульфана в группе А были снижены по сравнению со стандартными в 69% случаев. Ожидаемые значения AUC значимо коррелировали с наблюдаемыми значениями, и точность прогноза на основе тестовой дозы была оценена как 101,9±17,9%. Частота ВОБ в группе А была 3,4% по сравнению с 24,1% в группе Б, частота появления стоматитов была практически одинаковой. Приживление трансплантата было успешным у всех пациентов группы А. Частота полного приживления в течение 3 месяцев после пересадки была выше в группе А (p=0,012). Выживаемость в течение длительного периода наблюдения не отличалась в сравниваемых группах, в отличие от 907дневной выживаемости. Статистическое сравнение периодов времени (анализ выживаемости) без развития тромботических осложнений продемонстрировало преимущества в группе А (p=0,026).
